Additional Insights Gained From Implementation of Expanded Cough Icd-10-Cm Codes

This study examined how expanded ICD-10-CM cough codes, introduced in FY 2022, changed the ability to distinguish cough types in administrative claims data. Before FY 2022, cough had only one code (R05), but the new system includes acute cough (R05.1), subacute cough (R05.2), chronic cough (R05.3), cough syncope (R05.4), other specified cough (R05.8), and unspecified cough (R05.9).<br /><br />Researchers analyzed inpatient admissions in the Vizient Clinical Data Base from FY 2020 to FY 2022 and identified 154,353 admissions with a cough diagnosis. Among the new codes, unspecified cough was most common (68.7%), followed by chronic cough (26.1%) and other specified cough (3.9%).<br /><br />Demographic patterns differed by code. Most cough categories were roughly balanced by sex, but cough syncope was predominantly male (76.2%). Cough syncope also had a higher proportion of Non-Hispanic Black patients than the other cough categories. Average age varied, with subacute cough patients being the oldest on average (65.2 years) and unspecified cough patients the youngest (58.0 years).<br /><br />Clinical outcomes and reasons for care also differed. Acute cough was most often associated with respiratory infections and inflammation, subacute cough with respiratory signs and symptoms, and chronic cough with septicemia or severe sepsis. Acute cough had the highest in-hospital mortality (3.0%) and the longest average length of stay (9.1 days), while subacute cough had the lowest mortality (1.3%) and shortest stay (6.6 days).<br /><br />Overall, the expanded cough codes provide more clinically meaningful detail and reveal important differences in patient characteristics, utilization, and outcomes that were hidden when all coughs shared a single code.
