Addiction Consult Service Outcomes Among Patients With Alcohol Use Disorder

This study evaluated whether an inpatient Addiction Consult Service (ACS) improves treatment and short-term outcomes for hospitalized patients with alcohol use disorder (AUD). The authors conducted a retrospective, propensity score-matched historical control study of 215 admissions with ACS consultation matched to 215 similar historical controls. The ACS team included an addiction medicine attending physician, fellow, and peer advocate.<br /><br />The main outcomes were inpatient provision of medications for alcohol use disorder (MAUD, such as naltrexone or acamprosate) and MAUD prescribed at discharge. Secondary outcomes included patient-directed discharge, 30-day all-cause readmission, and 30-day post-discharge emergency department (ED) visits.<br /><br />Patients in the consulted and control groups were similar in most demographic and clinical characteristics after matching. ACS consultation was associated with a marked increase in evidence-based AUD medication use:<br />- Inpatient MAUD: 1.4% in historical controls vs 40.5% in consulted admissions<br />- MAUD at discharge: 1.9% vs 47.4%<br /><br />However, ACS consultation was not associated with statistically significant differences in patient-directed discharge, 7-day or 30-day readmission, or 7-day or 30-day post-discharge ED visits.<br /><br />The authors conclude that ACS consultation substantially improves delivery of evidence-based AUD pharmacotherapy during hospitalization and at discharge, suggesting important value for hospitals caring for many patients with AUD. They note that this was a single-center observational study focused on short-term care outcomes, and future research should examine linkage to outpatient care, long-term medication continuation, alcohol use outcomes, and mortality.
