Addiction Consult Service: Inpatient Outcomes in Opioid Use Disorder

This study evaluated whether an inpatient Addiction Consult Service (ACS) improves treatment for hospitalized patients with opioid use disorder (OUD). Although medications for OUD (MOUD) reduce mortality, they are often underused.<br /><br />Using a retrospective propensity-score matched design, researchers compared 100 admissions that received an ACS consult with 100 historical control admissions. The groups were similar in demographics, clinical characteristics, and hospitalization factors after matching.<br /><br />Key findings showed that ACS consultation was associated with substantially higher MOUD use during hospitalization: 57% of consulted admissions received inpatient MOUD compared with 34% of controls. Buprenorphine use in the hospital was also much higher with ACS (26% vs 6%). At discharge, buprenorphine prescriptions were far more common in the consulted group (26% vs 2%). In addition, 22% of consulted admissions had buprenorphine continued within 60 days, and 7% were admitted to a methadone program within 30 days of discharge, whereas these outcomes did not occur in the control group.<br /><br />There were no significant differences between groups in patient-directed discharge, 30-day emergency department visits, or 30-day readmissions.<br /><br />Overall, the study suggests that an Addiction Consult Service markedly improves initiation of MOUD in the hospital, buprenorphine prescribing at discharge, and linkage to ongoing outpatient treatment. The authors conclude that ACS implementation may be especially valuable in settings with a high prevalence of OUD because of its potential to improve access to life-saving treatment.
