Adapting a Mentored Implementation Model for a Multi-Site Copd Transitions of Care Quality Improvement Study

This document describes the adaptation of the Society of Hospital Medicine’s Mentored Implementation Model (MIM) for a multi-site COPD transitions-of-care quality improvement study, REVISITS. REVISITS is an NIH-funded, type II hybrid effectiveness-implementation cluster randomized trial evaluating COPD transition-of-care bundles across diverse U.S. hospitals, with the goal of reducing COPD revisits and identifying scalable, sustainable solutions.<br /><br />The implementation support model paired trained physician mentors with site leads to guide COPD transition-of-care interventions such as medication reconciliation and inhaler education. Mentors received at least 7 hours of training through SHM’s Mentor University and related virtual sessions, plus ongoing monthly mentor-only calls and resources for monthly meetings with site teams. Support was delivered virtually and, when needed, in person. Half of the sites also received co-design support.<br /><br />The adaptation was successful and highly acceptable. Twelve mentors were recruited across two cohorts and assigned to sites; some mentors supported more than one site. Monthly mentor-site meetings were conducted with 99% adherence (242/244 meetings), and all 20 planned site visits were completed. Evaluations showed strong positive feedback: mentors reported high confidence, readiness, and expertise after training, and site leads reported very high levels of support and value from mentor engagement. Specifically, site leads rated overall mentor support at 100%, and 97% found the mentored support valuable.<br /><br />Overall, the findings suggest that virtual training, ongoing mentoring, and structured monthly support can effectively adapt the MIM framework to support implementation of COPD transitions-of-care programs across multiple hospitals, with potential to improve care for chronic conditions.
