Acute Postcardiac Injury Syndrome Following Pci: A Rare Case of Rapid-Onset Pericardial and Pleural Effusions

This case report describes a rare, rapid-onset postcardiac injury syndrome (PCIS) following percutaneous coronary intervention (PCI). PCIS is an inflammatory condition affecting the pericardium and pleura after cardiac injury, most commonly seen after cardiac surgery and rarely after PCI.<br /><br />An 85-year-old woman presented with acute chest pain and shortness of breath. She was afebrile but hypertensive, anemic, and had elevated troponin with ST elevations on EKG, raising concern for STEMI. She underwent cardiac catheterization, which revealed a severe mid-LAD lesion treated with a drug-eluting stent. The procedure was complicated by LAD dissection. Immediately afterward, she became hypotensive and developed pleuritic chest pain.<br /><br />Further evaluation showed leukocytosis, a new left pleural effusion on chest X-ray, and a large loculated pericardial effusion on echocardiogram with right ventricular collapse. She was diagnosed with PCIS secondary to PCI. Treatment included colchicine and pericardiocentesis, which drained 200 mL of fluid. After the effusion resolved and the drain was removed, she was discharged in stable condition.<br /><br />The report emphasizes that PCIS should be considered in patients who develop pericardial or pleural effusions after recent cardiac procedures, even when symptoms appear acutely rather than weeks later. Early recognition and treatment are important to reduce morbidity and improve outcomes.
