Acute Kidney Injury in Patients With Coronavirus Disease 2019: A Single Center Experience

This single-center study examined acute kidney injury (AKI) among hospitalized COVID-19 patients at Lyndon Baines Johnson Hospital in Houston, Texas, between March 1, 2020 and January 29, 2021. Of 1,217 eligible patients, 615 were included after excluding those with unknown baseline creatinine, end-stage renal disease, or transfer out of the hospital. AKI was defined as a creatinine increase of at least 50% within one week.<br /><br />The study compared demographics, comorbidities, medication exposures, laboratory values, renal replacement therapy, and mortality between patients with and without AKI. Statistical analyses included Fisher exact tests, t tests, and logistic regression.<br /><br />AKI occurred in 138 of 615 patients, or about 22.4%. Patients with AKI were older, more likely to be male, and had higher mortality than non-AKI patients. Comorbidities such as hypertension and type 2 diabetes were common, and hypertension was significantly associated with AKI. Among all measured conditions, hypertension and sepsis were the strongest predictors of AKI in logistic regression, with sepsis showing the largest effect.<br /><br />Several medication and illness exposures were also assessed, including IV contrast, beta-lactams, TMP/SMZ, ACE inhibitors/ARBs, spironolactone, IV heparin, PPIs, NSAIDs, tenofovir, remdesivir, and sepsis. Sepsis stood out as a major associated factor. Patients with AKI also had more abnormal renal-related urine findings, including hematuria, pyuria, and proteinuria.<br /><br />Among AKI patients, 52 individuals (38%) required hemodialysis. Renal recovery, defined as return to baseline creatinine within 20%, occurred in an average of 9 days, though the recovery range was wide.<br /><br />The authors concluded that AKI is common in COVID-19 and is linked to increased mortality, especially in patients with hypertension, sepsis, and other renal-risk comorbidities. They noted that sepsis may be an important confounder and called for further research into the mechanisms and prevention of COVID-related AKI.
