Acceptability and Early Impact of Group Versus Individual Hospitalist Quality Incentive Program Metrics

This study examined whether hospitalists prefer individual or group quality metrics in an incentive program, and whether the metric type affects performance improvement.<br /><br />A survey of 121 hospitalists at UCLA (80% response rate) found that both individual and group metrics were generally acceptable. Preferences for individual metrics were not associated with gender, race, ethnicity, years in practice, work engagement, burnout, or involvement in quality improvement or service, though hospitalists involved in committees were more likely to prefer an individual metric.<br /><br />The hospitalist quality incentive program launched in July 2024 with two metrics: one individual metric, timely discharge summaries, and one group metric, goals-of-care documentation. Over six months, group performance improved significantly on both measures. Discharge summaries signed within 48 hours increased from 85.4% to 94.9%, and goals-of-care documentation rose from 35.8% to 62.8%.<br /><br />When individual performance was examined, 83.3% of hospitalists improved or maintained performance on the individual metric, compared with 67.2% on the group metric. This difference was statistically significant.<br /><br />Overall, the findings suggest that both individual and group metrics can be acceptable and can motivate behavior change in hospitalists. However, individual metrics may drive improvement more effectively for some clinicians. The authors conclude that these differences should be monitored over time and across multiple metrics to determine whether the effect persists.
