A Twisted Case of Colon Cancer

This case describes a 60-year-old man who presented with two months of right lower quadrant abdominal pain, nausea, vomiting, weight loss, and intermittent hematochezia. Lab work showed elevated inflammatory markers and microcytic anemia. Initial CT imaging suggested a possible ascending colon mass or intraluminal hematoma, and a three-phase CT confirmed ileocolic intussusception.<br /><br />Because adult intussusception is uncommon and often caused by an underlying lesion, surgery was pursued rather than colonoscopy, which was considered high risk for perforation. Exploratory surgery revealed a cecal mass causing the intussusception. The patient underwent a right hemicolectomy with ileocolic anastomosis.<br /><br />Pathology confirmed invasive mucinous adenocarcinoma of the colon, staged pT3pN0. The patient was discharged with outpatient oncology follow-up.<br /><br />The report emphasizes several key teaching points: adult colonic intussusception is rare, accounts for a small fraction of adult intussusception cases, and is frequently associated with malignancy. In adults, symptoms may be nonspecific and differ from the classic pediatric presentation. Mucinous colorectal adenocarcinoma is an important malignant cause, often arising in the right colon, frequently presenting at advanced stage, and associated with a poorer prognosis and potentially reduced chemotherapy responsiveness.<br /><br />Overall, the case highlights that adult intussusception should prompt strong suspicion for cancer and further diagnostic evaluation, especially in the setting of abdominal pain, anemia, weight loss, or gastrointestinal bleeding.
