A Teaming Approach to Improve the Morbidity & Mortality Conference

This document describes a quality improvement initiative that used the “teaming” framework to redesign Morbidity and Mortality (M&M) conferences in an internal medicine residency program at Mayo Clinic. The authors note that although M&M conferences are common in residency education, their impact on both learning and patient safety is inconsistent. Because reviewing safety events involves variability and unpredictability, they applied “teaming,” a dynamic approach suited to complex, changing tasks.<br /><br />The project organized collaboration among the QI and Patient Safety (QIPS) Council, internal medicine residents, chief residents, and faculty. The process was divided into several components. In the “scoping” phase, the team identified needed expertise, collaborators, and responsibilities. In “structuring,” they created standardized tools such as a case-scoring sheet and a timeline for stakeholder interviews. In “sorting,” faculty mentors helped identify institutional experts before residents began interviews. The team also emphasized purpose by aligning the redesign with two goals: improving patient care and teaching quality improvement principles.<br /><br />The initiative also focused on the “software” aspects of teaming. To build psychological safety, they created discussion spaces where residents and attendings could openly share concerns and ideas. To embrace failure, they improved documentation by creating an online file-sharing system and assigning a meeting secretary. To put conflict to work, they encouraged diverse perspectives from both frontline clinicians and leaders to identify improvement opportunities.<br /><br />The authors conclude that using a dynamic teaming approach can successfully improve both education and patient care, and may help identify broader systems issues for future improvement.
