A Team Based Approach to App Onboarding in Hospital Medicine

This document describes a 12-week, team-based onboarding program for new physician assistants (PAs) in a hospital medicine service at an 800-bed academic medical center. The program was created to address the challenge of training advanced practice providers (APPs) effectively while making efficient use of staff and resources.<br /><br />The program’s goals were to develop competent providers capable of safe, effective patient care within three months, support shared clinical/didactic/simulation learning, onboard multiple APPs at once, and use existing resources efficiently. The training team included three trainees, two experienced APP trainers, and one attending physician.<br /><br />Training was structured to gradually increase patient assignments over eight weeks until trainees reached a full census. Education included daily rounds with bedside teaching, short daily chalk talks on clinical and non-clinical topics, rapid response simulations, and self-directed learning through the AAPA/SHM Boot Camp on Demand course. Trainees also maintained logs based on the Journal of Hospital Medicine Core Competencies framework. Progress was monitored through weekly evaluations and formal feedback meetings at weeks 4, 8, and 12.<br /><br />Results showed that the team-based model helped trainees reach full patient census faster than standard one-on-one training. It also reduced the average number of required trainers by 25%, from 12 to 9. Post-training survey feedback was highly positive: 100% of trainees and 95% of trainers were “very satisfied,” and all trainees felt team training was more beneficial to professional development.<br /><br />The authors conclude that team-based onboarding is an effective model for APP training in hospital medicine and plan to continue and expand it to other departments.
