A Retrospective, Propensity-Score-Matched Analysis of the Outcomes and Effect on Length of Stay of a Newly Created Hospitalist Medicine Procedure Service

This retrospective study evaluated whether creating a Hospitalist Medicine Procedure Service (HMPS) for paracentesis and thoracentesis reduced hospital length of stay (LOS) compared with interventional radiology (IR) at two Grand Rapids hospitals from April 2021 to May 2023.<br /><br />Using propensity score matching to balance patient characteristics and procedure type, the authors compared LOS and time from order placement to procedure completion between groups. They also used regression analysis to identify factors associated with LOS.<br /><br />The HMPS was associated with a significant reduction in LOS, averaging about 1 day saved per patient. Given more than 1,500 patients treated annually, this could translate to roughly 1,500 hospital days saved and an estimated $1.3 million in yearly cost savings based on hospital finance estimates. Complication rates between HMPS and IR did not differ significantly, suggesting that the new service did not meaningfully increase adverse events despite anticipated training-related inexperience.<br /><br />A key finding was that the time from order to procedure completion, rather than whether the procedure was done by medicine or IR, was most strongly linked to shorter LOS. Use of anticoagulants and antiplatelet medications was associated with longer LOS.<br /><br />Overall, the study suggests that implementing a hospitalist procedural service can improve efficiency and reduce inpatient stay without significantly increasing complications.
