A Resident-Led Intervention Improves Estimated Discharge Date (Edd) Accuracy

This quality improvement project evaluated whether resident-led interventions could improve the accuracy of estimated discharge dates (EDD) documented in the electronic health record and strengthen communication between residents and case managers. At baseline, EDD documentation was inconsistent: case managers updated the EHR field for only about half of patients, and residents were largely unfamiliar with EDD tools.<br /><br />Using LEAN methodology, the team identified gaps and introduced several interventions, including resident education, social media and flyer awareness campaigns, gamification with monthly prizes, email reminders, and automated weekend pager reminders. They also promoted use of an interdisciplinary communication tool embedded in the EHR.<br /><br />The goal was to raise EDD accuracy from 45% to 63% for at least two-thirds of intervention months. Over eight months, EDD accuracy improved by 17%, exceeding the target for more than half of the intervention period. By the end, 88% of residents reported verbally communicating EDD during interdisciplinary rounds, and 83% updated EDD in the EHR at least 1–2 days per week. Case managers also reported improved engagement: 88% said they updated EDD most or all days, and 88% used provider notes to help predict discharge dates.<br /><br />Interviews with case managers suggested that a more accurate and visible EDD created a shared disposition goal, helped identify barriers earlier, and was especially useful when residents updated EDD after hours or on weekends.<br /><br />Overall, the study concluded that resident-led, interdisciplinary interventions can improve EDD accuracy and foster a stronger shared understanding of discharge planning in the hospital.
