A Qualitative Study of PHM Leaders' Perceptions of the 2025 Acgme Pediatric Residency Changes on the Pediatric Hospital Medicine Workforce

This qualitative study explored how Pediatric Hospital Medicine (PHM) leaders perceived the impact of the 2025 ACGME pediatric residency changes on the PHM workforce. Prior to implementation at their institutions, PHM physician leaders participated in focus groups using a semi-structured interview guide. The researchers analyzed the discussions with a hybrid inductive/deductive thematic approach and reached consensus on key themes.<br /><br />Leaders anticipated that the ACGME changes would affect PHM in several ways. First, they expected increased direct clinical responsibilities for hospitalists, including more coverage of general and subspecialty patient care. While this could expand hospitalists’ expertise and create new service opportunities, it would also likely reduce teaching time with residents. Participants felt this reduction could negatively affect resident recruitment into PHM, limit opportunities for academic advancement, and decrease career satisfaction for hospitalist educators. At the same time, they noted that hospitalists may gain opportunities to teach other learners.<br /><br />Another major theme was concern about preparing future physician hospitalists. Leaders were uncertain whether residents would receive enough inpatient exposure to be ready for PHM careers and emphasized the need to evaluate how fellowship training and other mitigation strategies can address potential gaps. They also discussed the role of advanced practice providers (APPs), recognizing their potential to strengthen PHM teams while also noting challenges in onboarding, oversight, recruitment, and retention.<br /><br />Overall, leaders viewed the changes as both a challenge and an opportunity. They emphasized that PHM’s adaptability and history of resilience are strengths, but also stressed the need for intentional workforce planning, advocacy, and collaboration at local and national levels to protect and shape the future of the specialty.
