A Novel Collaborative Hospitalist-Led Pharamcy Transitions in Care Rotation

This document describes a novel, hospitalist-led transitions-of-care pharmacy student rotation developed in partnership with the UF School of Pharmacy at the University of Florida. The six-week rotation pairs fourth-year pharmacy students with admitting hospitalists to improve medication reconciliation, identify medication errors, and clarify discrepancies across outpatient pharmacies, family reports, and outside facilities.<br /><br />The program was piloted in two cycles and is currently in a third. In Cycle 1 (Fall 2020–Spring 2021), the rotation was adapted to telemedicine because of the COVID-19 pandemic. One student worked with hospitalist attendings to complete admission medication reconciliation in the electronic medical record, which was then reviewed and verified by the hospitalist. Students also researched clinical pharmacology questions, presented journal club articles, and led patient case discussions. Feedback from students and hospitalists was collected at midpoint and end of the rotation and used to refine the experience.<br /><br />In Cycle 2 (Fall 2021–Spring 2022), the rotation returned to in-person learning and expanded to more students. The scope also broadened to include discharge planning, discharge medication review, and identification of social barriers to safe discharge in collaboration with case management. Students contributed meaningfully by contacting families and pharmacies to build accurate medication lists, especially for complex patients. The rotation was again rated highly, with students describing it as one of their most valuable experiences.<br /><br />The authors conclude that while pharmacist-led medication reconciliation is known to improve care, not all institutions have multidisciplinary transition-of-care programs. A hospitalist-led pharmacy student rotation offers a practical academic model that enhances patient safety, supports interprofessional education, and provides valuable teaching and clinical learning opportunities.
