A Hospital Medicine Resident Elective Curating Medical-Education and Quality-Improvement Experiences

This document describes a compressed 2-week hospital medicine elective for internal medicine residents designed to address gaps in training in quality improvement (QI) and medical education (Med Ed). Although ACGME expects residents to gain exposure to these domains, most programs offer limited opportunities, and standard hospital medicine electives are often 4 weeks long, making them hard to fit into busy residency schedules.<br /><br />The elective includes one week of clinical hospital medicine mentoring plus one structured, hands-on nonclinical week focused on either Med Ed or QI. Residents are paired with a hospital medicine faculty mentor from a pool of QI- or Med Ed-focused volunteers. The nonclinical week is highly structured over 4 days.<br /><br />In the Med Ed track, residents complete a resident-as-teacher exercise. They select a teaching topic and format, develop visual aids and content with iterative mentor feedback, rehearse the session, and then teach 3–6 medical students in person. Feedback is provided throughout on clarity, organization, pacing, interactivity, and visual design.<br /><br />In the QI track, residents perform a root cause analysis (RCA) for an assigned patient case using multiple tools, including fishbone diagrams, cause mapping, the 5 Whys, and process mapping. They also conduct a “gemba walk” by shadowing non-physician hospital staff to better understand workflow, then refine their process map and identify possible interventions using an action priority matrix.<br /><br />The authors conclude that this 2-week elective offers meaningful QI and Med Ed training in a shorter, more feasible format than traditional 4-week rotations. Its strengths include a low resident-to-faculty ratio, structured daily plans, personalized mentorship, and iterative feedback. The rotation has been well received by both residents and faculty.
