A Case of Daptomycin Associated Eosinophilic Pneumonia

This case describes daptomycin-associated eosinophilic pneumonia, a recognized adverse effect of the antibiotic daptomycin, which is commonly used for gram-positive infections such as MRSA and VRE. The reaction is an important consideration in patients treated for bone and joint infections, where reported incidence is 1.7–4.8%.<br /><br />A 67-year-old man with peripheral arterial disease and chronic left lower extremity wounds was hospitalized for osteomyelitis. Based on intraoperative culture results, he was started on daptomycin. Eight days later, he developed shortness of breath and hypoxia, requiring supplemental oxygen. His symptoms worsened and peaked 11 days after starting the drug, at which point he needed 15 L/min of oxygen via nonrebreather mask.<br /><br />Laboratory testing showed leukocytosis with eosinophilic predominance, supporting the diagnosis. Imaging also demonstrated bilateral pulmonary opacities on chest X-ray and bilateral upper lobe ground-glass opacities on CT chest. Daptomycin was discontinued, and the patient improved after treatment with steroids. Over the following two weeks, he was gradually weaned off oxygen.<br /><br />Daptomycin-induced eosinophilic pneumonia typically presents 2–4 weeks after starting therapy and is characterized by fever, dyspnea, and hypoxia. Diagnosis is often confirmed by bronchoalveolar lavage showing eosinophils, though peripheral eosinophilia may also support the diagnosis. Prognosis is generally favorable when the condition is recognized early. Main treatment includes stopping daptomycin, providing supportive care, and administering glucocorticoids.
