This case report describes a 26-year-old woman with a history of migraines who presented with a 3-day persistent occipital headache that was different from her usual migraines. Her symptoms included blurred vision, neck pain, left ear throbbing, photophobia, painful eye movements, and visual field deficits. She also reported recent head trauma. Physical examination showed severe obesity (BMI 51.49) and elevated blood pressure, but no focal motor deficits.<br /><br />Because of the atypical headache and concerning associated symptoms, imaging was performed. CT head revealed acute cerebral sinus thrombosis, and CTA confirmed left transverse sinus thrombosis without hemorrhage. MRI and MRV showed thrombosis involving the left transverse sinus, left sigmoid sinus, superior internal jugular vein, and torcula. She was started on intravenous heparin and transferred to a tertiary center.<br /><br />At the receiving hospital, her headache persisted but her visual symptoms improved. The team opted for conservative management, and her hospital course remained uncomplicated. She was discharged home on apixaban with outpatient follow-up.<br /><br />The report emphasizes that cerebral venous thrombosis is a rare but important cause of stroke, accounting for about 0.5–1% of all strokes, and is more common in young women. Risk factors include obesity, head trauma, pregnancy, oral contraceptive use, malignancy, and hypercoagulable states. Since headache is the most common presenting symptom, clinicians should consider CVT in patients with new or atypical headaches plus risk factors or neurologic/visual symptoms to ensure early diagnosis and treatment.
