A Big Sib Peer Mentorship Program to Support New Hospitalists

This document describes a low-cost, low-effort “Big Sib” peer mentorship program created to support new hospitalist faculty and fellows during their first teaching-service block. New hospitalists often need real-time help with teaching, giving feedback to learners, handling complex cases, and learning institutional workflows. To address this, the program paired new faculty (“Little Sibs”) with more experienced teaching-service faculty (“Big Sibs”) through a personalized email introduction. Big Sibs were given general guidance to answer clinical, systems, and learner-related questions and mostly provided ad hoc support.<br /><br />Program evaluation showed positive results. Among first-year academic faculty and fellows surveyed, 55% responded. Of those, 81% reported being paired with a Big Sib during their first teaching block. Most met with their mentor one or two times, while some met more often and a few never met. The majority felt the program created a meaningful connection to another hospital medicine faculty member, and 76% said it was helpful in addition to other structured support. Nearly all respondents (94%) thought the program should continue. Big Sibs also reported positive experiences, noting the time commitment was minimal and that they enjoyed helping new colleagues.<br /><br />The authors conclude that the Big Sib program was successfully implemented and was valued by both mentors and mentees. It appears to be a useful complement to orientation, coaching, and formal mentorship programs, and it may be easy to adopt at other institutions. A challenge was keeping track of when new faculty first rotated onto service, especially when schedules changed.
