Video Summary
This video was a hospital medicine webinar on dermatology emergencies and inpatient rashes. The speakers reviewed hypersensitivity reactions using the Gell and Coombs classification, emphasizing timing, morphology, and when a rash is “sick or not sick.” They covered:

- Type I reactions: urticaria, angioedema, and anaphylaxis, including key features like rapid onset and transient wheals.
- Type II reactions: autoimmune blistering diseases such as pemphigus vulgaris and paraneoplastic pemphigus.
- Type III reactions: serum sickness and cutaneous small-vessel vasculitis, often presenting as palpable purpura.
- Type IV reactions: delayed T-cell mediated eruptions such as contact dermatitis, morbilliform drug eruptions, fixed drug eruption, DRESS, and Stevens-Johnson syndrome/toxic epidermal necrolysis.

The second half focused on recognizing dermatologic emergencies by where tissue is dying: epidermis, dermis, deeper soft tissue, or blood vessels. Important red flags included painful rash, mucosal involvement, targetoid lesions, Nikolsky sign, rapidly progressive ulcers, necrotic/violaceous borders, severe pain out of proportion to exam, and retiform purpura. Conditions discussed included SJS/TEN, mucormycosis, pyoderma gangrenosum, necrotizing fasciitis, vasculitis, vasculopathy, and calciphylaxis.

The speakers also answered practical questions about when to call dermatology, emphasizing that concerning or painful rashes should prompt urgent evaluation, and discussed urgent management of necrotizing fasciitis and calciphylaxis.
