Video Summary
This webinar from the Society of Hospital Medicine focused on designing effective backup (“Jeopardy”) systems for hospitalist groups. Speakers shared experiences from large academic and community divisions, describing how backup systems evolved from informal phone trees to structured, mandatory schedules with compensation, tracking, and shared governance. A major theme was that resistance often comes not to backup itself, but to concerns about fairness, overuse, staffing shortages, and lack of input from frontline clinicians.

Presenters emphasized best practices: engage leadership early, compensate clinicians for standby time and call-ins, use data to dispel myths of abuse, and involve frontline staff in design. They also highlighted organizational justice—making systems transparent, equitable, and respectful.

Examples included division-wide backup coverage, proportional participation by clinical FTE, and alternative strategies when backup wasn’t feasible for APPs or nocturnists. The panel also discussed how telemedicine, flexible work, moonlighters, and proactive staffing can reduce backup burden.

The qualitative research presented found Jeopardy systems are common but highly variable, often associated with dissatisfaction due to uncertainty, inadequate compensation, and perceived unfairness. Overall, the panel concluded that backup systems are necessary, but they work best when they are fair, clearly communicated, and thoughtfully implemented.
