Video Summary
The Society of Hospital Medicine webinar examined how hospitalists can advocate for compensation by explaining their value in terms hospital leaders recognize.

The speakers contrasted professional billing—where hospitalists generate revenue through coded encounters and work RVUs—with hospital facility payments, which may depend on fee-for-service, diagnosis-related groups (DRGs), quality incentives, bundled payments, or accountable care arrangements. These models can reward accurate documentation, quality improvement, efficient care, and lower total costs, not simply higher patient volume.

A hospitalist director presented ways a team can contribute: improving safety and patient experience, reducing length of stay and readmissions, strengthening multidisciplinary rounds and care coordination, improving documentation, supporting appropriate staffing, and contributing to surgical volume and organizational goals. The administrator emphasized financial pressures facing hospitals, including rising costs, unpaid care, insurance-related administrative burdens, and payment risk.

The discussion outlined three common compensation components—base pay, productivity, and performance incentives—and described shift-based, salary, and shared-revenue models. Performance measures should be meaningful, measurable, achievable, and manageable. In the example negotiation, the director proposed a compensation increase tied partly to quality and citizenship incentives, higher RVU rates, and better billing performance supported by dashboards and training.

The speakers encouraged hospitalists to build shared goals with administrators, demonstrate measurable impact, and plan compensation adjustments proactively rather than framing negotiations as adversarial. They also addressed questions about subsidies, advanced practice clinicians, administrative costs, productivity, and unionization.
